Visit your doctor or midwife and ask for a blood test to confirm that you are pregnant. If the results return positive, then you can make a second appointment to discuss your pregnancy and receive your pregnancy declaration forms, or formulaires de déclaration de grossesse.
Depending on your medical history, you may be provided with your declaration forms at this appointment. If you have a history of miscarriages or other health concerns, you may be asked to do an ultrasound before the forms are given to you.

The forms should be filled out and mailed before you reach your 14th week of pregnancy. The sooner they are mailed, the sooner you are covered by France’s social security (if you qualify for social security).
The pink form needs to be mailed with your blood test results to your Caisse d’Assurance Maladie, and the two identical blue forms should be mailed to your Caisse d’Allocations Familiales.
